A joint WHO and UNICEF report finds only 40% of health care facilities worldwide met basic sanitation standards in 2025, with 11% having no sanitation services at all, affecting 936 million people. The gap was widest in sub-Saharan Africa, where only 23% of facilities met the standard, compared with 55% in Central and Southern Asia.

The same report found gaps across water, hygiene and cleaning: 85% of facilities had basic water access, but only 59% met environmental cleaning standards requiring both protocols and trained staff, a figure that fell to 24% in the world's least developed countries.
