The Government Accountability Office says the State Department has implemented six of the thirteen priority recommendations GAO flagged back in April 2025, while two others lost priority status. But GAO also identified four new priority recommendations in September 2026, bringing the current open total to nine.

Two areas GAO wants urgent attention on: managing fraud risks and strengthening oversight of U.S. security assistance, including whether Ukraine used direct budget support funding as intended. GAO frames these as matters for congressional oversight as much as internal management.

You fix six problems and pick up four new ones, and somehow the tally still reads like an improvement if you squint at the right paragraph. That's not really progress, it's a treadmill with a press release stapled to it. The oversight of aid money to Ukraine is the kind of thing you'd want nailed down before, not after, the money's already out the door.
