VA Health Care: Action Needed to Improve Fertility Care Communications and Eligibility Determination Process

A GAO review finds the number of veterans diagnosed with infertility rose from about 19,000 in fiscal 2022 to roughly 30,000 in fiscal 2025, out of roughly 8 million veterans using VA health care, an increase VA officials attribute partly to 2024 eligibility expansions. But GAO found the VA's main channel for communicating about fertility care runs through the Office of Women's Health, leaving male veterans, who also experience infertility, less likely to seek out the information.

In October 2024 the VA shifted eligibility decisions for fertility care from national-level review to individual medical center fertility teams, but GAO found team composition and skill levels varied by facility, officials described the eligibility calls as challenging, and two facilities failed to consistently send veterans the required written eligibility notifications.

GAO recommended VA adopt standard communication performance practices and assess whether the new eligibility process is actually working; VA agreed with both recommendations.

Handing veterans a webpage filed under women's health and calling it outreach is a bit like putting the fire exit sign in the basement and calling the building safe. The VA agreed with every recommendation, which is the easy part; agreeing costs nothing until somebody checks back next year. A veteran who doesn't know the door exists can't be blamed for not walking through it.
