At least 20 transgender women are estimated to be held across ICE facilities in Arizona and Colorado, a number obscured by federal policy denying trans people's legal existence under a Trump executive order. Most are housed in men's units rather than according to gender identity, and interviews describe near-total denial of medically prescribed hormone therapy following policy rollbacks at DHS, DOJ and the Federal Bureau of Prisons since last year.

One detainee, Valentina, described nine days in isolation after an accusation she denies, and 22 days without hormone therapy that left her experiencing physical changes worsening her dysphoria. Medical experts interviewed say abrupt hormone withdrawal can trigger menopause-like symptoms, severe psychological distress and spiking suicidal ideation, particularly for those on long-term therapy, though research on forced discontinuation remains limited.
