Midterm Ads Clash Over GOP Law’s Impact on Rural Healthcare

Ads in House and Senate races are dueling over the One Big Beautiful Bill Act's effect on rural healthcare, with Republican-backed spots in Iowa, Colorado, Alaska and Ohio celebrating a new $50 billion Rural Health Transformation Program while omitting the law's much larger Medicaid cuts. Those cuts are projected to reduce rural funding by $137 billion over a decade, and hospitals alone could lose $57 billion, according to KFF and a Manatt analysis commissioned by the National Rural Health Association.

Democratic ads go the other direction, warning of hospital closures and citing specific facility counts that experts say are impossible to predict with precision. The Congressional Budget Office projects the law will cut federal Medicaid spending by about $900 billion over ten years and push 10 million people off insurance, 7.5 million of them from Medicaid.

The rural health fund itself is a one-time, five-year pot meant for new programs like AI tools and chronic-disease prevention, not for covering uncompensated care or existing services. A Georgetown Medicaid expert noted the fund has real value but 'far exceeds' nothing compared to the losses it's meant to offset.
